Annual Health and Medical Record: ALL participants in basic Scouting activities such as day camps, local tours, and weekend camping trips less than 72 hours in duration must complete this medical record. Ensure that you complete the entire form (parts A, B1, and B2). A copy of the record will be confidentially maintained with the leader of the activity. This must be updated at least annually.
